Size Collection Form
Click the link below to create your own copy of the shirt size collection form. Follow these steps to set it up for your chapter:
- Open the link—you’ll be prompted to make a copy of the form.
- Click 'Make a copy'—this will save a version of the form to your Google Drive.
- Rename the form to include your chapter and event name for easy identification (e.g., 'Alpha Phi Bid Day Shirt Size Form').
- Customize the form—edit any questions as needed to match the options available for your order (e.g., remove unavailable sizes or add specific preferences).
- Test the form—click the eye icon (Preview) at the top to review how it looks for your members.
- Publish and share the form link:
- Click the Publish button at the top-right corner.
- In the Manage responders section, change the responder access to ‘Anyone with the link.’
- Click Done and Publish, then select the link icon at the top that says ‘Copy responder link.’
- This is the link you’ll send to your members!
Tip: You can easily track responses later by going to the Responses tab in the form and clicking ‘Link to Sheets’ to view and organize submissions in a spreadsheet.
