Size Collection Form

Click the link below to create your own copy of the shirt size collection form. Follow these steps to set it up for your chapter:

 Google Form

  1. Open the link—you’ll be prompted to make a copy of the form.
  2. Click 'Make a copy'—this will save a version of the form to your Google Drive.
  3. Rename the form to include your chapter and event name for easy identification (e.g., 'Alpha Phi Bid Day Shirt Size Form').
  4. Customize the form—edit any questions as needed to match the options available for your order (e.g., remove unavailable sizes or add specific preferences).
  5. Test the form—click the eye icon (Preview) at the top to review how it looks for your members.
  6. Publish and share the form link:
  • Click the Publish button at the top-right corner.
  • In the Manage responders section, change the responder access to ‘Anyone with the link.’
  • Click Done and Publish, then select the link icon at the top that says ‘Copy responder link.’
  • This is the link you’ll send to your members!


Tip: You can easily track responses later by going to the Responses tab in the form and clicking ‘Link to Sheets’ to view and organize submissions in a spreadsheet.
